Every consequential action in an enterprise leaves evidence behind it. A wire transfer has an approval chain. A trade has a ticket. A claim denial has a reviewer and a reason code. These records exist because someone always asks why, and "the system did it" has never been an acceptable answer.

AI agents are now taking actions at that level of consequence, in insurance, finance, healthcare, legal and critical infrastructure. Most of them leave nothing comparable behind. Logs show what ran. They do not show why the action was justified.

When the question arrives, and it will arrive, the answer that holds up is a decision record. A useful one has five properties:

  • Bounded evidence. The record shows exactly what the agent knew at decision time. Not the whole database. The evidence the decision was allowed to use.
  • The policy that applied. Not a gesture at values. The customer's own policy, at the version that was in force, applied to the evidence.
  • A deterministic verdict. Allow, block or hold, produced by code, not by another model. A verdict that can be rerun and reproduced.
  • The human in the loop. Where the workflow required a person, the record shows who approved and what they saw.
  • Independent verifiability. Anyone holding the record can check it offline, without access to the system that produced it and without taking that system's word. This is the property that matters to a regulator, an LP, a board or a litigation team, because it is the property that makes the record evidence rather than testimony.

Runtime-containment platforms answer a different question. They stop an agent from leaving its sandbox in the moment. They built the cage. The cage does not testify. When someone external asks why the agent did what it did, you need the record.

This is the problem Forge Orbital is built for. Forge is the accountability layer for AI agents: deterministic code applies your policy, every decision leaves a record, and the record can be checked offline by the people who will ask. One synthetic proof trail and its offline verifier are public on this site for exactly that reason. Do not take our word for it. Check the record.
